Abstract
Mesenchymal stem cells (MSCs) are mature stromal cells that can differentiate into several cell types. MSCs is the most attractive regenerative medicine due to its anti-inflammatory, immunosuppressive and tissue repair properties. Recent studies have shown that through their paracrine activity MSCs function better, most of their work is carried out by extracellular vesicles (EVs). The extracellular vesicles of MSCs (EVs-MSCs) can resemble their parent functions by delivering their component contents such as microRNA, mRNA, DNA, organelles, lipids and proteins to the recipient cell. This review article was written based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The purpose of this article is to review the effectiveness of EVs-MSCs-assisted miRNAs for the improvement of liver disease. This review article also elaborate the concept of EVs, miRNA library, MSCs EVs in liver disease. Treatment of EVs-MSCs in various liver diseases can decrease collagen types I and III, reduce elevated serum alanine aminotransferase(ALT) levels, serum aspartate aminotransferase(AST) and production of pro-inflammatory cytokines such as IL-6, TNF-α, interferon-γ(IFN-γ), IL-1β and IL-18 as well as altering growth factor TGF-β1 and inhibiting epithelial to mesenchymal transition (EMT). miRNA modification can increase the therapeutic potential of AMSCs in the treatment of carbon tetrachloride (CCl4)-induced rat liver fibrosis by inhibiting activation of Liver stellate cells (HSCs) and suppressing collagen deposition. Several identified miRNAs from EVs MSCs are miRNA 122, miRNA 181-5-p and miRNA 223 with correlation to improved liver disease treatment outcome. In conclusion, miRNA modification mediated by EVs-MSC hold the potency to improve the outcome of liver disease treatment.
